Medicine is answered as clinical vignettes, not definitions: a stem gives a presentation and asks for the next investigation, the diagnosis or the first drug. Cardiology, endocrinology, pulmonology and hematology-oncology carry most of the weight. The efficient route is to learn each condition as a mechanism that predicts its signs, so that an unfamiliar stem can still be reasoned through, and to know the Indian epidemiology that decides which diagnosis is likely in the first place.
