Medicine is answered as clinical vignettes, not definitions: a stem gives a presentation and asks for the next investigation, the diagnosis or the first drug. Cardiology, endocrinology, pulmonology and hematology-oncology carry most of the weight. The efficient route is to learn each condition as a mechanism that predicts its signs, so that an unfamiliar stem can still be reasoned through, and to know the Indian epidemiology that decides which diagnosis is likely in the first place.

A liver test panel tells you whether the hepatocytes or the ducts are injured long before it tells you the cause, and a gastrointestinal presentation tells you whether the problem is in the lumen, the wall or the motility. 4Hematology & OncologyHaematology is answered by a fixed sequence - the count localises, the indices narrow, the smear characterises, and one confirmatory test settles it. Oncology is answered by a different question - what is the intent of treatment, and is there an emergency that must be dealt with before the cancer is addressed at all. 5Infectious DiseasesAcute undifferentiated fever in India has a short and predictable list, and the fever itself contributes almost nothing to distinguishing between its members. 6NephrologyThree questions asked in order place almost any renal presentation - is it acute or chronic, is the problem before the kidney, within it, or after it, and which compartment is involved. 7PulmonologyOne ratio splits respiratory medicine in half. An obstructive pattern and a restrictive pattern have almost no diseases in common, so establishing which one is present eliminates most of the differential before any imaging is considered. 8Rheumatology & Autoimmune DiseasesThe pattern of joint involvement makes the diagnosis and the antibody confirms it, never the other way round. Which joints, how many, whether the distribution is symmetrical, and whether the pain is inflammatory or mechanical together narrow the differential to a handful of conditions before any serology is sent.⏱ 21 min read
